OVERVIEW

Documents implementation of community skills development and rehabilitative skills training for adults with intellectual and developmental disabilities at local activity centers. Responsible for developing plans and conducting reviews for community coaching, community engagement and group day services. Work is performed under general supervision and is guided by agency policies and procedures, quality assurance, public and private payor guidelines, professional practices and including ethical standards related to client interactions, state and federal licensure and accreditation regulations and guidelines.

PRIMARY

DUTIES AND RESPONSIBILITIES
  • Conducts group meetings and participates in psychological developmental meetings with other staff and the client participants.
  • Dispenses medication.
  • Provides crisis intervention and supportive counseling.
  • Sets up tours and provides program information to prospective participants and their families or other interested parties.
  • Monitors program goal completion and enforces program rules.
  • Monitors and dispenses medications.
  • Transports or arranges for member transportation.
  • Assists individual with medical, financial, social, hygiene and recreational activities.
  • Makes collateral contacts with the parents, guardians, providers, friends, and siblings of the persons served to promote implementation of the services plan and supports provided.
  • Trains staff on medication administration procedures, case documentation, use of the electronic health record system, assistive technology and personalized equipment, member coaching.
  • Monitors program operations, peer audits, staff performance and documentation completion for compliance to licensing requirements and quality control standards.
  • Monitors compliance to state and local licensure standards.
  • Monitors facility safety and security, maintain health and safety manual and conducts safety drills and staff training.
  • Completes assessments, admissions, and discharges to and from services.
  • Prepares documentation of services for Intellectual Disabilities Waiver, DD waiver, sponsors, OBRA, ICF, private pay.
  • Develops Individual Service Plans for a caseload of individuals served and oversees implementation of the plan.
  • Assesses individuals for pre-discovery/volunteer activities for career exploration.
  • Completes quarterly Person Centered Reviews of each individual's service plan to accurately reflect current status and progress toward goals.
  • Reviews the quarterly Person Centered Reviews with each individual and his/her authorized representative and documents satisfaction, changes requested, and provides follow‑through as needed.
  • Submits and manages authorizations in the Virginia Waiver Management System (WaMS).
  • Will be cross trained in the role of Day Support Site Leader and will be utilized in that capacity to meet the daily requirements of the program.
SECONDARY RESPONSIBILITIES AS NEEDED
  • Prepares service and activity reports for billing.
  • Schedules, provides supervision and evaluates day support staff at multiple locations.
  • Schedule and implement specified services of community coaching, community engagement and group day services.
  • Rotates availability for on‑call support for after‑hours program on weekends.
  • Schedules and conducts new employee orientation.
  • Arranges for the maintenance and repair of program equipment to include vans, office machines, adaptive equipment, video surveillance equipment etc. and maintains related records. Reports building and grounds issues.
  • Maintains inventories, requisitions/purchases facility supplies and program budgets for multiple locations.
  • Schedules and/or reviews all community outings and on‑site activities.
  • Develops and maintains the staffing schedule and approves staff leave.
KNOWLEDGE, SKILLS, AND ABILITIES
