• Develop and execute strategic marketing plans to grow referrals
• Build and maintain relationships with hospitals, physicians, SNFs, assisted living communities, and case managers
• Identify new referral sources and expand territory reach
• Conduct in-person sales calls and community outreach
• Track referral activity and maintain CRM documentation
• Collaborate with clinical and intake teams to ensure smooth transitions of care
• Meet and exceed monthly census growth goals

• Minimum 3 years of home health marketing experience (required)
• Proven track record of growing census and referral networks
• Strong understanding of Medicare home health guidelines
• Excellent communication and relationship-building skills
• Self-motivated, goal-driven mindset
• Valid driver’s license & reliable transportation
