OVERVIEW

Provides nursing oversight to programs for intellectually disabled () and developmentally disabled (DD) persons; including Group Homes, Intermediate Care Facilities (ICF), Supervised Apartment Programs, Sponsored Placement, and Day Support Programs. Work involves observing, assessing and administering nursing care to residents; staff training; and supervision of medical aspects and needs of individuals in programs. Work is performed under general supervision and is guided by agency policies and procedures, quality assurance, public and private payer guidelines, professional practices including ethical standards related to client interactions, Board of Pharmacy and Board of Nursing regulations, state and federal licensure and accreditation regulations and guidelines.

DUTIES

AND RESPONSIBILITIES OF POSITION
  • Provides medical oversight and general nursing care when needed.
  • Provides medication management, monitoring, and related documentation for compliance to agency, state and federal policies, regulations and/or guidelines.
  • Provides input to individual service plans in collaboration with the residents, their families, and doctors, as appropriate.
  • Coordinates care of individuals with program managers/supervisors.
  • Monitors and coordinates the discharge process for individuals transitioning back into residential services from in‑patient hospitalizations.
  • Conducts medication audits and chart reviews across DD Programs.
  • Supervises resident medication management, monitoring, and related documentation for compliance to agency, state and federal policies, regulations and/or guidelines.
  • Provides direct care, dispenses medications, and administers injections to residents.
  • Provides routine examinations such as measuring blood pressure, pulse, temperature, weight and blood glucose readings.
  • Submits billing for eligible nursing related waiver services.
  • Transports residents.
  • Monitors the on‑going medical needs and medication management for individuals when
  • Performs on‑going nursing assessments and arranges access for individuals to other medical
  • Provides trainings to include but not limited to Medication Management, Insulin, G‑Tube, and Diastat training to agency staff.
  • Develops and implements medical/nursing care plans and other clinical documentation as needed.
  • Responds to medical emergencies and other potential crisis situations in programs as needed.
  • Member of the ICF Personal Support team and coordinates care with the ICF Medical Director and Pharmacists.
  • Develops, implements, trains and enforces policy and procedures.
  • Attends required team meetings, staff meetings, and conferences.
  • Responds to time sensitive medical concerns, triage, and provide guidance to programs operating on a 24/7 basis.
  • Does related work as required.
KNOWLEDGE, SKILLS, AND ABILITIES
  • Thorough knowledge of the principles, standards, practices and techniques related to medical care, nursing, and treatment in service to the intellectually disabled or developmentally delayed population.
  • Thorough knowledge of the characteristics, administration and precautions of medications typically used by the population.
  • Thorough knowledge of Board of Pharmacy and Board of Nursing regulations relating to prescription drug storage, administration and documentation.
  • Thorough knowledge of human rights of residents.
  • Thorough knowledge of program development and maintenance, including state and local licensure policies and procedures; record‑keeping; supervision and evaluation.
  • Thorough knowledge of liability issues.
  • Thorough knowledge of service plan development, training and evaluation; ability to establish and maintain rapport with the individual population.
  • Ability to coordinate and schedule appointments, treatment services and follow up.
  • Ability to maintain effective working relationships.
  • Ability to communicate effectively, both orally and in writing.
  • Good fiscal and facility management skills.
  • Ability to plan the work of, train, supervise and evaluate subordinates.
  • Ability to communicate effectively orally and in writing.
