Human Touch Home Health is seeking an experienced Registered Nurse (RN) to provide skilled in-home nursing services throughout Fredericksburg and surrounding counties.

• Perform comprehensive patient assessments in the home setting

• Develop and implement individualized care plans

• Provide skilled nursing care including wound care, medication management, and disease education

• Complete accurate and timely OASIS documentation

• Educate patients and caregivers on disease management and safety

• Coordinate care with physicians and interdisciplinary team members

• Monitor patient progress and adjust care plans as needed

• Active Virginia Registered Nurse (RN) license

• Minimum 2 years nursing experience (home health preferred)

• OASIS experience preferred

• EMR experience (Well Sky/Kinnser or Homecare Homebase a plus)

• Strong clinical assessment and documentation skills

• Valid driver’s license & reliable transportation
