Job Summary

The Program Coordinator, Centering Pregnancy, is responsible for providing professional nursing care, following established standards and practices as part of the patient care team. This role provides leadership and operational oversight for Centering Pregnancy and/or Centering Parenting programs. This role serves as the Leader of the Centering Steering Committee and is responsible for program coordination, clinical support, evaluation, and administrative support to ensure high‑quality, sustainable Centering services.

In addition, the coordinator leads school health initiatives and manages resident scheduling for community health rotations, supporting broader community and population health goals.

Essential Functions & Responsibilities
  • Leads and coordinates day‑to‑day operations of the Centering program, including scheduling, patient coordination, meeting preparation, and program communications.

  • Oversee program resources, including monitoring and maintaining clinical supplies, educational materials, and equipment needed for Centering visits, school health activities, and related program operations.

  • Serves as the leader of the steering coming, and primary educator for group‑based prenatal care and owning patient education delivery.

  • Facilitates patient flow during Centering sessions and collaborates with providers, nursing staff, faculty, learners, and interdisciplinary teams to support continuity of care.

  • Maintains accurate records, attendance logs, databases, and required documentation in accordance with Centering program requirements.

  • Coordinates administrative and clinical workflows to ensure efficient, patient‑centered group care experiences.

  • Answers and triages patient nursing calls, responds to routine clinical questions, and communicates concerns to providers as appropriate.

  • Performs nursing‑related clinical support duties within scope and under appropriate clinical supervision, including obtaining vital signs, prescription refills, documenting patient information, preparing patients for visits, and assisting with care coordination activities.

  • Conducts nurse visits as delegated and appropriate within scope, including patient assessments, education reinforcement, follow‑up care, and documentation.

  • Coordinates patient intake, appointment coordination, chart preparation, follow‑up communication, prior authorizations, and clinical documentation support.

  • Provides and reinforces patient education related to wellness, preventive care, treatment plans, medications, and available community resources to support engagement and health outcomes.

  • Supports coordination and support of school health program activities, including communication with schools, students, families, and clinical staff, scheduling services, maintaining required documentation, and supporting school‑based health initiatives.

  • Serves as a liaison between patients, providers, faculty, learners, schools, and support departments to ensure effective communication and coordination.

  • Supports quality improvement initiatives, workflow development, and process enhancements to support program growth, patient engagement, and operational efficiency.

  • Performs other duties as assigned.

Qualifications

Required:

  • Valid LPN Licensure in the Commonwealth of Virginia required; RN candidates may be considered.

  • CPR certification required.

  • Minimum of two years of clinical experience required. Experience in women's health, obstetrics, maternal‑child health, or community health preferred.

  • Experience with computer technology and electronic health record software required.

Preferred:

  • Associate's degree in nursing preferred; current RN licensure preferred.

  • 3‑5 years of experience in program coordination, community health, population health, or healthcare operations or clinical experience.

  • Experience supporting group‑based care models preferred.

  • Experience working with community partners, schools, or public health agencies strongly preferred.

  • Experience coordinating academic or clinical training schedules (residents, students, or interns) desirable.
