Case Manager – CSAC-A/CSAC-S/CSAC About Conscious Healing

Conscious Healing is a trauma‑first substance use disorder treatment program serving individuals through PHP, IOP, OP, individual counseling, ASAP‑related services, and recovery support. Our mission is to help participants heal at the root and move toward lasting freedom through compassionate care, accountability, connection, and whole‑person support.

We are looking for a dependable and compassionate Case Manager to support participants with care coordination, resource connection, documentation, treatment engagement, and recovery stability.

Position Summary

The Case Manager works as part of the clinical and operations team to help participants identify needs, remove barriers to treatment, and connect with appropriate community resources. This role supports participants with housing, transportation, employment, legal, medical, insurance, recovery support, and discharge planning needs.

The ideal candidate is organized, professional, trauma‑informed, and comfortable working with individuals impacted by substance use, mental health challenges, trauma, legal involvement, and recovery‑related barriers.

Required Qualifications
  • CSAC-A or CSAC-S required
  • Bachelor’s degree required
  • Minimum 1 year of experience in behavioral health, substance use treatment, case management, human services, recovery services, or a related field
  • Strong documentation and communication skills
  • Ability to maintain professional boundaries and confidentiality
  • Ability to work as part of a team in a fast‑paced treatment environment
  • Knowledge of substance use disorder treatment, recovery resources, and community supports preferred
Responsibilities
  • Provide case management support to participants in PHP, IOP, OP, ASAP, individual services, and recovery support programming
  • Assist participants with identifying barriers to treatment and recovery stability
  • Connect participants to community resources, including housing, transportation, employment, medical care, benefits, legal resources, food assistance, and recovery supports
  • Support discharge planning, aftercare planning, and continuity of care
  • Coordinate with clinical staff, admissions, front desk, medical providers, probation/parole, ASAP, hospitals, residential programs, and community partners as appropriate
  • Complete case management documentation accurately and within required timelines
  • Participate in treatment team meetings and communicate participant needs to the appropriate team members
  • Help participants follow through with appointments, referrals, paperwork, and recovery goals
  • Maintain participant confidentiality in accordance with HIPAA, 42 CFR Part 2, and company policy
  • Support crisis escalation procedures by notifying clinical staff or leadership when safety concerns arise
  • Maintain professional, ethical, and trauma‑informed interactions with participants, families, staff, and referral partners

Compensation

$20.00 – $23.00 per hour
, based on experience, credentialing, and qualifications.

Schedule

This is a part‑time position, 25+ hours per week
. Daytime availability is required, with some flexibility based on program and participant needs.

Work Environment

This position is based in a substance use disorder treatment setting. The Case Manager may interact with participants experiencing emotional distress, crisis, relapse risk, housing instability, legal stressors, and other recovery‑related challenges. The position requires professionalism, confidentiality, teamwork, and strong boundaries.
