Social Worker (Program Coordinator)
The Social Worker (Program Coordinator) works for the Fredericksburg Health Care System (FxHCC) in Fredericksburg, VA, as a direct report to the Patient Aligned Care Team (PACT) and Specialty Outpatient Programs Social Work Supervisor.
