Groups Recover Together is seeking a Recovery Support Coordinator in Fredericksburg to support a panel of 160-180 members. You will coordinate care across clinical and community settings, engage members, and help remove barriers to care in a hybrid work model.
You will collaborate with clinicians, social services, and community partners, performing outreach, coaching, and care coordination to promote sustained recovery and wellness across the member journey.
