Community Outreach Case Manager – Suicide Prevention

• Create partnerships and linkages and identify individuals needing services
• Coordinate Veterans Program outreach efforts and develop outreach plans
• Visit VA facilities, shelters, and other locations where veteran families congregate
• Communicate and provide updates to the VA and community providers
• Screen potential participants for eligibility using VA-approved forms
• Verify veteran and income status with applicable agencies
• Provide outreach, mental health screening, suicide prevention education, emergency clinical services, case management, peer support, and benefits assistance
• Arrange wraparound services and connect clients with VA and mainstream resources
• Communicate confidentially with the service team to address service and support issues
• Participate in supervision, staff and Project Management Team meetings, trainings, and collaborative planning
• Assist people experiencing homelessness with resource access, referrals, shelter, and permanent housing
• Maintain computerized client records, daily activity logs, mileage logs, and reports
• Operate and maintain a van/shuttle when needed, including inspections and cleaning
• Build trust through frequent outreach visits
• Research community resources benefiting veterans and advocate for veterans facing barriers
• Foster integration with homeless services, veteran services, behavioral health, and other programs
• Travel throughout the community and local sites
• Provide community stabilization support and immediate mental health services during crises
• Help decrease hospitalization utilization, law-enforcement contact, and recidivism

• Bachelor's degree in Human Services or related field required
• 3 to 5 years of experience in outreach
• Experience working with Veterans or people with mental health, substance use, or IDD concerns
• Ability to explain an agency's purpose, programs, and operations
• Knowledge of available community resources
• Mission-driven self-starter committed to helping people connect with services
• Excellent computer skills in Microsoft Office and office products
• Excellent written and verbal communication skills
• Proven ability for conflict resolution
• Ability to safely operate a motor vehicle
• Ability to travel to assigned locations and perform required physical activities
• Bilingual in any language is a plus

Demonstrates expertise in outreach coordination, community resource navigation, and mental health support for veterans. Proficient in case management, conflict resolution, and maintaining accurate client records while fostering partnerships with various service providers.
