- Assist with resident assessment and implementation of care needs and will participate in resident and family care conference
- Must be able to effectively communicate with residents, families, staff, physicians, outside providers and the public
- Ensures that changes in resident condition are evaluated, monitored, documented and effective interventions are implemented
- Provide support and assistance to medication management systems
- Ensure that medications are dispensed in a safe and effective manner, keep medication cart orderly, and all medications appropriately packaged and stored
- Monitor resident health concerns including weight and nutrition, skin integrity, mobility, fall management and pain management
- Current licensure as a Practical Nurse (LPN) or Registered Nurse (RN) in the State of New Jersey
