Position Overview

BAYADA Home Health Care is seeking a Registered Nurse, RN for a full‑time position with our new Central NJ Senior Living team – a Medicare‑certified home health office that provides best‑in‑class care to adult and geriatric clients living in assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities across Somerset, Middlesex, and Monmouth Counties
. The role may also include seeing clients in the traditional home health community setting.

• Follow a designated care plan in accordance with patient needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients’ conditions and report changes to the Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors’ orders
• Perform client assessments as necessary
• Case‑manage and coordinate care
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state‑of‑the‑art tablet

• A current license as a Registered Nurse in New Jersey
• Minimum one year of recent, verifiable clinical (medical/surgical) experience
• Prior home‑care experience strongly preferred, but not required
• Graduation from an accredited nursing program, as indicated by transcript or diploma
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills; prior experience with electronic medical records (EMR) preferred
• Ability to travel to cases as assigned

$58.00 – $70.00 per point, depending upon qualifications, experience, and subject to negotiation.
