Job Summary
2 weekends per month on Saturdays with on‑call on Sundays.
Position located at FHN Memorial Hospital, Freeport, IL 61032. Salary range $35.00 - $57.75 per hour. Job type:
Ad Lib/Per Diem/As Needed.
- Evaluate patients for initiation, change or discharge of plan of care.
- Develop and implement treatment plans incorporating treatment methods and goals with patient/caregiver input.
- Complete relevant documentation and record‑keeping services and consult with other disciplines per regulatory guidelines.
- Design, fabricate, fit and educate in use of assistive devices, special equipment, splints and orthotics.
- Select or adapt appropriate programs and activities tailored to patients’ needs consistent with their physical and mental ability, enabling them to reach their full potential and return to a prior level of function or adapted environment.
- Perform home/work site evaluation when appropriate with recommendations to enable patient to return to a safe environment.
- Consult with internal and external team members, disciplines or services to provide plan of care and recommendations to progress patient rehabilitation.
- Educate or instruct patients, family members, caregivers, staff or team members of skills and techniques required for maximum functional outcome.
- Attend and participate in customer care, staff conferences and other meetings.
- Develop and implement training programs for other healthcare members on Physical Therapy objectives and techniques.
- Prepare, clean and ensure proper working order of patient care equipment; report repair needs to appropriate department per policy.
- Supervise support personnel in accordance with state and professional standards.
- Adhere to FHN policies, procedures and PRIDE Standards.
- Display competency for age‑specific patients: pediatrics to geriatrics where applicable.
- Participate in process improvement/quality indicator tracking when applicable.
- Display competency for the Physical Therapist role as evidenced in annual performance appraisal following FHN PRIDE Standards.
- Supervise Physical Therapist Assistants, Rehabilitation Technicians, Students, and Volunteers.
- Other duties may be assigned.
- Education:
Doctorate or Master’s Degree in Physical Therapy preferred;
Bachelor’s Degree from a four‑year college or university in Physical Therapy considered with relevant experience. - Licensure/Certification/Registration:
Current licensure or eligible as a PT under the Illinois Physical Therapy Act. - Maintain AED/BLS certification.
Requires frequent walking, standing, and reaching. Must lift up to 75 lbs (patient) and perform repetitive hand/arm and foot/leg movements. Must be able to climb, squat, kneel, and sit as required.
