You will work closely with our interdisciplinary care team to create personalized care plans, conduct resident assessments, and implement rehabilitation programs to promote independence and mobility. Your expertise will also be essential in identifying and addressing potential health issues, providing education and support to residents and their families, and maintaining accurate and detailed records.
