• 1.0 FTE (5-day work week) with flexible day‑off scheduling to support work‑life balance
• Rural outpatient‑only practice with a loyal, established patient panel
• Minimal on‑call responsibilities
• Patient‑centered scheduling averaging at least two patients per clinical hour, allowing time for relationship‑based care

• Provide comprehensive outpatient primary care services to patients across the lifespan
• Conduct patient evaluations including detailed health histories and physical examinations
• Diagnose and manage acute and chronic medical conditions
• Develop and implement individualized treatment plans in collaboration with the supervising physician
• Order, interpret, and follow up on diagnostic tests and lab results
• Prescribe medications and therapies in accordance with state regulations and best practices
• Promote preventive care and wellness, including screenings and immunizations
• Educate patients and families on disease management and healthy lifestyle choices
• Maintain accurate and timely documentation within the EMR
• Collaborate with care teams and refer patients to specialists as needed

• Strong clinical skills with the ability to work both independently and collaboratively
• Excellent communication and interpersonal skills
• Commitment to compassionate, patient‑centered, evidence‑based care
• Interest in practicing in a rural healthcare setting

• Graduate of an accredited Nurse Practitioner program
• Current Illinois APRN license (or eligibility)
• DEA license (or ability to obtain)
• NP experience strongly preferred
