Mercyhealth, a community-based healthcare network serving southern Wisconsin and northern Illinois, is seeking an Internal Medicine Nurse Practitioner to join our team at Mercyhealth Burchard Hills Clinic in Freeport, IL
.
- Join a highly respected Internal Medicine team within a multi-specialty practice focused on adult and geriatric care
- Full‑time position with a strong emphasis on work‑life balance
- Compensation range: $95,280-$144,976
- Practice with clinical autonomy while supported by robust administrative and operational resources
- Focus on adult and geriatric populations, managing chronic conditions and preventive health
- Light 1:16 call schedule (telephone only)
- Collaborative team environment with experienced Internists and Nurse Practitioners
- Supportive, mission‑driven culture with long‑tenured providers and strong community ties
- Provide comprehensive care to adult and geriatric patients in an outpatient setting
- Conduct detailed health histories, physical examinations, and risk assessments
- Diagnose and manage acute and chronic medical conditions
- Develop and implement individualized treatment and care plans
- Order, interpret, and follow up on diagnostic tests and lab results
- Prescribe medications and therapies in accordance with state regulations
- Coordinate care with specialists and other members of the healthcare team
- Promote preventive care, wellness, and chronic disease management
- Educate patients and families on health conditions and treatment plans
- Maintain accurate and timely documentation within the EMR
- Master's Degree or Doctorate (DNP) in Nursing from an accredited institution
- Certification as a Nurse Practitioner (FNP or AGNP)
- Strong clinical and critical‑thinking skills with the ability to work independently and collaboratively
- Excellent communication and interpersonal skills
- Commitment to compassionate, patient‑centered, evidence‑based care
- Interest in practicing in a community‑based or rural healthcare setting
- Current Illinois APRN license (or eligibility)
- DEA license (or ability to obtain)
