Mercyhealth, a community-based healthcare network serving southern Wisconsin and northern Illinois, is seeking an Internal Medicine Nurse Practitioner to join our team at Mercyhealth Burchard Hills Clinic in Freeport, IL
.

  • Compensation range: $95,280-$144,976
  • Practice with clinical autonomy while supported by robust administrative and operational resources
  • Focus on adult and geriatric populations, managing chronic conditions and preventive health
  • Light 1:16 call schedule (telephone only)

Responsibilities
  • Provide comprehensive care to adult and geriatric patients in an outpatient setting
  • Conduct detailed health histories, physical examinations, and risk assessments
  • Diagnose and manage acute and chronic medical conditions
  • Develop and implement individualized treatment and care plans
  • Order, interpret, and follow up on diagnostic tests and lab results
  • Prescribe medications and therapies in accordance with state regulations
  • Coordinate care with specialists and other members of the healthcare team
  • Promote preventive care, wellness, and chronic disease management
  • Educate patients and families on health conditions and treatment plans
  • Maintain accurate and timely documentation within the EMR
Qualifications
  • Master's Degree or Doctorate (DNP) in Nursing from an accredited institution
  • Certification as a Nurse Practitioner (FNP or AGNP)
  • Strong clinical and critical‑thinking skills with the ability to work independently and collaboratively
  • Excellent communication and interpersonal skills
  • Commitment to compassionate, patient‑centered, evidence‑based care
  • Interest in practicing in a community‑based or rural healthcare setting
Licensure / Certifications
  • Current Illinois APRN license (or eligibility)
  • DEA license (or ability to obtain)
