As an LPN on our evening shift, you will be responsible for providing skilled nursing care to our residents, including:

• Administering medications and treatments as prescribed by physicians

• Monitoring and reporting changes in residents' conditions

• Assisting with daily living activities, such as bathing, dressing, and grooming

• Providing wound care and management

• Working closely with the healthcare team to develop and implement individualized care plans

• Provide direct patient care to residents on the evening shift, including administering medications, taking vital signs, and performing basic nursing procedures

• Conduct daily rounds to assess resident conditions and report any changes to the healthcare team

• Develop and implement individualized care plans in collaboration with the healthcare team

• Maintain accurate and timely records of resident care and treatment
