Part‑time Registered Nurse Case Manager

Saturday / Sunday

Utilize your clinical skills to complete the initial assessment of patients to determine home care needs.

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow‑ups and evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to the Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided

• Current and unrestricted CA RN license
• Current BLS/CPR certification
• Valid CA driver’s license
• Experience conducting assessments and Start of Care (SOC) – OASIS preferred
• High level of clinical knowledge in home health care preferred
• Experience with high-acuity patients a plus
• Experience working with an interdisciplinary team preferred
• Case Management fundamentals desired
• Previous experience with IV medication infusions preferred
• Proficiency in initiating intravenous therapy a plus
• Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, organized
