- The Clinical Nurse provides direct patient care, supports chronic disease management, and collaborates with providers and staff to ensure efficient clinic operations.
- This role involves performing health assessments, administering medications, documenting patient records, and participating in outreach initiatives to promote preventive health behaviors.
- Direct Patient Care:
Perform health assessments, screenings, immunizations, wound care, and medication administration as prescribed by providers. - Chronic Disease Management:
Educate patients on managing chronic conditions and promote preventive health practices. - Collaboration & Support:
Work alongside providers, behavioral health specialists, and staff to coordinate patient care and assist with clinic workflow. - Documentation:
Maintain accurate electronic medical records (EMR) in compliance with safety and regulatory standards. - Outreach & Counseling:
Conduct telephone triaging, patient counseling, and participate in community health initiatives.
- Active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the State of California.
- Previous experience in a community health center, primary care, or outpatient setting preferred.
- Strong communication skills and ability to work independently in a fast-paced environment.
- Competency in handling diverse patient populations.
- Must have at least 2 years of recent experience in the specialty within the past 2 years.
- CPR/BLS Certification required.
