Overview
The Case Manager is a member of our Housing Team and is responsible for providing crisis intervention and case management services to survivors of Intimate Partner violence who are served in our Mariposa 2 Rapid Re-housing program. The Case Manager carries an ongoing caseload of approximately 15-20 households.
Ideal CandidateStrong candidates for this position will demonstrate the ability to thrive in a fast-paced, crisis services environment. The right candidate will be warm and outgoing and enjoy interacting with the public. The case manager must have the ability to demonstrate empathy and compassion even in difficult circumstances. The case manager must be able to juggle multiple tasks and demonstrate skill in prioritization and organization.
They must enjoy working independently as well as being part of a larger team.
Previous experience providing case management services to survivors of trauma in a non-profit, direct services organization is strongly desired. Previous experience in HUD funded programs and demonstrated knowledge of HUD guidelines and requirements is strongly desired.
There is a preference for candidates that speak a language (in addition to English) that is commonly spoken within the client populations we serve - examples include Spanish, Hindi, Farsi, Mandarin and Arabic. Candidates who are also bi-cultural and who have lived experience as a survivor are preferred.
Responsibilities- Provide intensive crisis intervention and case management services to survivors who are eligible for housing assistance and subsidies from SAVE.
- Provide housing assessments, housing problem solving services and other housing placement related activities in accordance with HUD guidelines and local Alameda County Gender Based Violence Survivor Coordinated Entry System (GBV/CE) guidelines.
- Maintain all GBV/CE required documents in the database.
- Attend required GBV/CE meetings, participate in related trainings and work to build relationships with partner agencies and providers.
- Assist survivors to apply for and obtain benefits for which they are eligible (SSI/SSDI, CalWORKS, VOC). Provide necessary advocacy with governmental agencies.
- Assist survivors in obtaining the documents required in order to demonstrate eligibility for housing opportunities.
- Conduct individual case management sessions with survivors to identify needs, set goals, make referrals, and track survivor progress.
- Assist survivors to identify and enroll in school and/or employment training programs that will enhance economic stability.
- Provide ongoing case management, including: safety planning, risk assessment, and short-term and long-term intervention planning.
- Identify obstacles and risks to the survivors' safety and stability, and work with survivors to overcome these obstacles and mitigate the risks.
- Provide regular mobile or in-home advocacy and case management as appropriate.
- Provide client transportation in agency vehicle and/or personal vehicle.
- Provide follow-up/after-care case management services for survivors who are no longer receiving a subsidy.
- Develop and/or conduct workshops on topics relevant to program participants (budgeting, time management, credit repair).
- Maintain a directory of resources that would benefit program participants.
- Meet with the program manager for supervision regularly and as needed.

- Respond to crisis calls and walk-in survivors.
- Track services provided and client information in the electronic database on a daily basis. Ensure electronic client files show current, updated information and changes in demographics.
- Attend all required meetings, including staff meetings, trainings, consultations, and retreats.
- Assist with training new staff members and volunteers.
- Assume responsibility for site security and safety as needed.
- Provide input into program planning and developments.
- Assist with maintaining program operations and office functions as needed.
