We provide care across long-term care and ort-stay rehab, focusing on getting residents back to their own homes safely and efficiently. As a PRN/On-Call Physical Therapist, you’ll have the flexibility to work when it fits your schedule while making a real difference in the lives of those recovering from illness or surgery.

• Evaluate residents and develop personalized treatment plans to address functional deficits, mobility needs, and rehabilitation goals.
• Provide direct therapy services in accordance with physician orders and industry standards.
• Collaborate with an interdisciplinary team—including nurses, physicians, and other therapists—to ensure holistic resident care.
• Educate residents, families, and caregivers on therapeutic techniques and strategies to promote independence and safe mobility.
• Accurately document evaluations, progress notes, and treatment outcomes in compliance with regulatory and facility requirements.

• Current Physical Therapist license in the state of Nebraska (or eligibility).
• Degree in Physical Therapy from an accredited program.
• Strong clinical and communication skills, with the ability to motivate and encourage residents.
• Previous experience in a skilled nursing, assisted living, or rehabilitation setting preferred.
