Physical Therapist – Full-Time | New Grads Welcome!

Join our vibrant rehab team in a state-of-the-art facility designed for comfort, healing, and excellent patient care. We provide independent living and assisted living (AL/IL) communities, memory care, outpatient therapy, and respite care.

As a physical therapist with us, you’ll work closely with a collaborative care team in our AL/IL outpatient therapy setting to develop personalized treatment plans, guide residents through therapy, and celebrate their progress along the way. New grads are encouraged to apply—we love mentoring and supporting the next generation of rehab professionals.

We are seeking a dedicated Physical Therapist (Full-Time) to join our rehabilitation team. The ideal candidate will provide high-quality, evidence-based care to residents across a variety of care settings. You will work closely with our interdisciplinary team to assess, plan, and implement individualized treatment plans that improve mobility, strength, and overall quality of life.

• Evaluate and treat residents to improve functional mobility, balance, strength, and endurance.
• Develop and implement individualized care plans based on assessment findings.
• Monitor and document patient progress, adjusting treatment plans as necessary.
• Educate residents and families on exercises, safety, and health management.
• Collaborate with nurses, occupational therapists, speech therapists, and other team members to provide comprehensive care.
• Assist with discharge planning and support residents in returning home whenever possible.
• Participate in staff meetings, in-services, and continuing education opportunities.

• Doctor of Physical Therapy (DPT) and current Nebraska Physical Therapy license.
• New graduates welcome to apply; mentorship and support provided.
• Strong interpersonal, communication, and documentation skills.
• Ability to work independently and as part of a collaborative team.
• Experience in long-term care or short-stay rehab is a plus.
