Licensed Clinical Social Worker (FT) – New Directions is the Geriatric Behavioral Health Unit for Dodge County Hospital. Hours to be discussed at interview.

The Social Worker is responsible for providing social service functions in a behavioral health care setting. Provides assessments, treatments, and case management to patients and families. Is a resource person to healthcare professionals concerning psycho-social problems and community resources. Maintains performance improvement activities within the department.

• Master’s degree in social work or clinical nurse specialist for psychiatric mental health
• Current licensure such as LCSW or LPC; experience in psychiatric setting preferred
• Knowledge of community resources for older persons, Medicare, Medicaid and insurance requirements for continued care
• Familiarity with medical terminology and conditions as these impact treatment and discharge needs
• Familiarity with medical team roles in hospital departments

• Clinical therapy responsibilities and care management, including treatment plans for patients
• Collect and evaluate data for social services and discharge planning
• Maintain an accurate and up-to-date knowledge of relevant regulations and standards regarding provisions of social services while implementing treatments and discharge planning
• Provide individual and group therapy, including family group therapy
• Communicate and update families when needed
• Complete all required assessments, such as Suicide Risk Assessment and Psycho-Social Assessment by the due date
• Assist in overseeing the intake of referrals and follow up with referral sources
• Participate in the weekly treatment team meeting and ensure appropriate follow-up documentation
• Ensure activity and socialization groups occur per schedule
