Overview:We are seeking a compassionate and experienced Registered Nurse – Case Manager to join our Home Health Care team. This role offers flexibility in scheduling while providing the opportunity to make a meaningful impact on patients’ lives.
Key Responsibilities:- Conduct comprehensive assessments and develop individualized care plans for patients in their homes.
- Coordinate and manage patient care, collaborating with physicians, therapists, and other healthcare professionals.
- Monitor patient progress, provide education, and adjust care plans as needed.
- Ensure compliance with all regulatory and clinical standards.
- Serve as the primary point of contact for patients, families, and caregivers.
- Current Registered Nurse (RN) license in Nebraska.
- Home health care experience is preferred but not required; new graduates are welcome.
- Excellent clinical assessment, communication, and organizational skills.
- Ability to travel within the North region to visit patients’ homes.
