Position Summary

Nye Home Health Care is seeking a compassionate and dedicated Part‑Time Licensed Practical Nurse (LPN) to join our team. In this role, you will provide skilled nursing care and support to patients in their homes, helping them maintain independence, improve health outcomes, and enhance their quality of life. The ideal candidate is patient‑focused, organized, and passionate about delivering exceptional care in a home health setting.

Essential Duties and Responsibilities
  • Provide direct nursing care to home health patients in accordance with physician orders, care plans, and agency policies.
  • Monitor and document patient conditions, treatments, and responses to care.
  • Administer medications and treatments as prescribed.
  • Assist with wound care, catheter care, and other skilled nursing procedures within scope of practice.
  • Educate patients and family members on health conditions, medications, treatments, and disease management.
  • Communicate patient status and changes in condition to the Registered Nurse (RN), physician, and interdisciplinary team.
  • Maintain accurate and timely clinical documentation.
  • Participate in care planning and coordination with healthcare providers and caregivers.
  • Follow infection prevention, safety, and regulatory standards at all times.
  • Support the organization's mission, values, and commitment to quality patient care.
Qualifications

Required:

  • Current Licensed Practical Nurse (LPN) license in the State of Nebraska.
  • Current CPR certification or ability to obtain upon hire.
  • Valid driver's license, reliable transportation, and proof of auto insurance.
  • Ability to travel throughout the assigned service area.
  • Strong communication, organizational, and time management skills.

Preferred:

  • Previous home health, hospice, long‑term care, or clinical nursing experience.
  • Experience working independently and managing patient visits.
