As a Hospice Registered Nurse (RN) Case Manager at Ohioans Home Healthcare & Hospice, you will play a key role in caring for patients and families within the Sandusky area territory. Your duties include assessing, planning, and implementing patient care; educating patients and families on comfort care processes, procedures, and medications; and addressing various patient needs.

Assess, plan, and implement patient care; educate patients and families on comfort care processes, procedures, and medications; monitor medication compliance, set‑up, IV therapy, injections; provide education on proper use of equipment; and offer continuity of care to patients and families.

• One year of comfort care experience.
• CPR certification.
