Family Nurse Practitioner

## Description Come join our team as a full-time Family Nurse Practitioner (FNP) at Community Health Services- 2221 Hayes Avenue, Fremont! This is an outstanding opportunity for an experienced, compassionate FNP, who enjoys caring for a diverse population of patients-including both adults and children. Applicants must have at least 3 years of experience in a family practice setting.$120,000 base salary, no RVU's and paid malpractice insurance.

As a Federally Qualified Health Center, our providers are eligible to apply for student loan repayment through National Health Service Corps (NHSC)- up to $75,000 for full-time employees. Other benefits include paid vacation, sick time, and retirement benefits.
** What You'll Do:**| Work directly with patients Monday through Friday
- No Weekends| Perform Adult and Pediatric Assessments| Communicate clearly with Providers, Patients, and CHS Staff| Follow-up on Lab and Diagnostic testing| Administer injections as needed| Follow all safety, HIPAA, and regulatory guidelines

* What We're Looking For:

**| Graduate of a current accredited Family Nurse Practitioner (FNP) Program| Current License to practice in the State of Ohio as a Family Nurse Practitioner (FNP)| Current Basic Life Support Certification| Minimum of three years as an FNP preferred| Knowledge of documenting with EHR| Strong communication skills and the ability to multi-task| Ability to critically think and problem solve| Reliable, professional, and compassionate
