Infection Control & Wound Nurse

Communi Care is seeking an Infection Control & Wound Nurse to lead infection prevention and wound care programs in our skilled nursing and long-term care setting. You will assess residents, develop and monitor wound treatment plans, track infection trends, conduct root-cause analyses, and ensure compliance with regulatory standards. You will educate staff on infection control practices, support survey readiness, collaborate with the interdisciplinary team, and champion evidence-based, resident-centered care in a compassionate, learning-focused environment with growth opportunities.

Responsibilities

• Lead infection prevention and control program across the facility
• Perform comprehensive wound assessments and develop evidence-based treatment plans
• Monitor and document wound healing progress and infection trends
• Conduct infection surveillance, reporting, and root-cause analysis for outbreaks
• Educate and coach staff on infection control and wound care best practices
• Ensure compliance with CDC, CMS, and state regulations and support survey readiness
• Collaborate with interdisciplinary team to optimize resident outcomes
• Participate in quality improvement initiatives related to infection and wound care
• Engage residents and families in care planning and education
• Maintain accurate, timely clinical documentation and reports
