Provider is an Internal Medicine or Family Medicine trained physician This physician will provide clinical services at the VACCHCS and provide leadership for their PACT Provider is expected to document into CPRS in a timely manner, participate in Process Improvement/Quality Assurance projects and participate in usual Medical Staff activity including committee participation Administrative duties are related to clinical activities pertaining to managing a panel of patients' health care, including preventative, ambulatory acute, and chronic disease management.
Provider is expected to maintain Joint Commission readiness.
Our Primary Care Physician (PCP) functions as a team provider in shared workflow with a multidisciplinary Patient-Aligned Care Team (PACT a medical home model), assigned to provide comprehensive, collaborative same day and continuity care to an assigned panel of Veterans as well as to patients outside of the assigned panel when Veteran care indicates a need for such. Medical care is delivered in accordance with current clinical practice guidelines with attention to prevention via utilization of evidence-based clinical decision-making tools.
In addition to the provision of high-quality medical care, PACT Physicians demonstrate excellence in customer service via compassionate and informed interpersonal relationships with Veterans, family members and staff.
