The Home Health Physical Therapist is responsible for delivering personalized rehabilitation services to patients in their homes, focusing on improving long-term health and wellness.

This role involves conducting comprehensive assessments, developing individualized treatment plans tailored to each patients goal

s.Responsibiliti

• es Conduct an initial evaluation/assessment and report findings to the physician / provider and Clinical Manager and develop the plan of care promptly for patien
• ts Perform physical therapy interventions, procedures, measurements, evaluations and diagnostic tests on patients in the hom
• eDocument initial evaluation and treatment plan and re-evaluations of patient’s progress every thirty (30) days and updates plan of care as necessa
• ry Complete comprehensive assessments and OASIS data, as indicated on a timely bas
• is Assist with orientation, supervision and consultation services to Physical Therapy Assistan
• ts Observe, record and report to Health Care Provider the patient’s treatment, response and progress and promptly report significant changes and obtain orders to revise plan of ca
• re Communicate with physician / provider and Health Care Provider and other members of the health care team for the delivery and coordination of quality physical therapy servic
