Candidate Pay Rate: $59.5 - $59.5 per hour

• Evaluate patients and develop individualized physical therapy treatment plans that improve function
• Deliver skilled therapy to enhance mobility, strength, balance, and independence
• Document evaluations, treatment sessions, and patient progress accurately and in a timely manner
• Collaborate with interdisciplinary healthcare teams to support optimal patient outcomes
• Educate patients and caregivers on therapeutic exercises, safety, and mobility strategies
• Maintain organized, safe, and compliant treatment areas
• Ensure adherence to facility policies, procedures, and state regulations

• Active California Physical Therapist (PT) license (required)
• Prior SNF or rehabilitation experience preferred
• Strong clinical judgment with excellent communication and documentation skills
• Ability to work independently and as part of a collaborative team
• Availability to commit to the full 13-week contract

• Patient evaluation and assessment techniques
• Individualized treatment planning and intervention
• Improving mobility, strength, balance, and functional independence
• Accurate and timely clinical documentation
• Interdisciplinary collaboration and clear communication with team members
• Patient and caregiver education for home safety and therapeutic carryover
• Familiarity with facility policies and state regulatory requirements
