Responsibilities
  • Assist in the accomplishment of the client’s case management plans through the provision of all program components and services. Services include individual services, and other forms of direct services in accordance with case management plans.
  • Provide intensive support to clients during the critical reintegration period. Services will include housing stabilization, transportation coordination, and linkage to health and social services.
  • Conduct needs assessments both in office and in the jail.
  • Prepare, review and update case management plans.
  • Make written notes on client interventions.
  • Document progress toward goals and maintain ongoing communication with Probation Officers.
  • Assist individuals in linking and accessing community resources, including housing assistance, substance use treatment, mental health care, and legal aid.
  • Work collaboratively with Job Developer to monitor progress and address any issues that hinder the participant from completing their goals.
  • Provide individual and group training on job readiness, résumé preparation, interviewing skills, job searching, transitional skills, and other job/education related topics.
  • Assess individuals for education and employment readiness. Determine employment, training and Career Technical Education (CTE) needs.
  • Develop individual employment plans (IEP).
  • Assist clients with analyzing/evaluating employment opportunities, personal abilities/aptitudes, and provide information on occupational opportunities. Discuss job requirements, training, rehabilitation resources and assist with formulating plans to achieve employment goals.
  • Mentor individuals in disclosing appropriate information regarding past convictions, and/or probation/parole status to potential employers.
  • Document services to meet all program, county, and state requirements.
  • Assist individuals with obtaining necessary documents to legally gain employment in the state of California (e.g., California , Social Security Card, etc.).
  • Advise individuals of the benefits of completing the criminal record expungement process.
  • Provide information, or a referral, on how to expunge a criminal record, and obtain a Certificate of Rehabilitation.
  • Provide the SUD Supervisor and/or Program Director with timely reports and updates including any key communications between community partners, vendors, clients.
  • Will drive on company business as needed.
  • Physical presence in the office is required.
  • Will drive on Agency business.
  • Will think and act quickly and efficiently in emergencies.
Qualifications
  • Possess either a bachelor’s degree in social sciences or a related field, or a high school diploma or equivalent and a minimum of two (2) years  experience working with a criminal justice or related population.
  • Certified training in emergency first aid procedures and CPR preferred.
  • Have a current AOD certification/license from a certifying body approved by the State of California Department of Health Care Services (DHCS) and maintain good standing OR currently registered to provide AOD counseling services from a certifying body approved by the State of California Department of Health Care Services (DHCS) and maintain good standing. (Preferred)
  • Intermediate computer skills in Microsoft Word, Excel, PowerPoint, and Outlook.
  • Basic computer skills in Office 365, Teams, Zoom and other communication/meeting platforms.
  • Ability to understand and use ARMS (Automated Reentry Management System); preferred.
  • Complete all required training for Turning Point, California Department of Corrections, West Care and Fresno County Probation
  • Ability to complete the minimum required hours of in-service training per year. This is a contractual requirement that could be subject to change.
