POSITION SUMMARY
Under the direction of the Director of Community Health, the Healthcare Partnership Specialist is responsible for developing and managing strategic relationships with healthcare providers, clinics, hospitals, and community health systems to generate referrals for the Comprehensive Asthma Remediation and Education (CARES) program. This position leads active territory development efforts, builds sustainable referral sources, and strengthens provider partnerships to expand program reach and impact.
The Healthcare Partnership Specialist serves as the primary liaison between healthcare partners and the organization, ensuring seamless referral processes and ongoing collaboration.
- Identify, prospect, and establish partnerships with hospitals, primary care practices, specialty clinics, FQHCs, school-based health centers, and other healthcare settings.
- Schedule and conduct meetings, presentations, and in-service trainings to educate healthcare providers and care teams on the CARES Program.
- Meet or exceed established monthly referral quotas to support program growth and sustainability goals.
- Develop and execute a strategic outreach plan to drive consistent referral volume.
- Track referral metrics, conversion rates, and partnership activity.
- Identify barriers to referrals and implement corrective strategies.
- Design and implement streamlined referral workflows directly from provider offices into the organization’s electronic health record (EHR) system.
- Collaborate with clinical teams to ensure referral processes are efficient, compliant, and user-friendly.
- Train provider offices on referral submission procedures and documentation requirements.
- Continuously evaluate and optimize referral pathways to improve ease of use and turnaround time.
- Create and update marketing materials, educational brochures, presentations, and referral guides tailored to patients and families, healthcare providers and community partners.
- Ensure materials are clear, culturally appropriate, and aligned with program standards.
- Develop comprehensive knowledge of asthma management best practices, environmental remediation strategies, and program eligibility criteria.
- Clearly articulate the clinical value, cost savings, and patient outcome improvements associated with the program.
- Relationship Building:
Demonstrates the ability to establish, nurture, and sustain long-term, trust-based partnerships with healthcare providers, clinical staff, and community organizations to drive consistent program referrals. - Strategic Outreach & Territory Management:
Develops and executes a structured outreach plan to identify high-potential partners, prioritize engagement efforts, and effectively manage a defined service area to maximize referral volume. - Results-Driven & Accountable:
Takes ownership of meeting and exceeding monthly referral quotas by proactively tracking performance metrics, identifying gaps, and implementing corrective strategies to ensure program sustainability. - Healthcare Delivery System Practices:
Possesses a strong understanding of healthcare delivery systems and clinical workflows to confidently establish relationships and referral pathways. - Process Development:
Designs, implements, and refines efficient referral pathways that integrate seamlessly with provider workflows and the organization’s electronic health record system. - Marketing & Communication
Skills:
Creates clear, compelling, and audience-appropriate educational materials and presentations that effectively convey the benefits and impact of the CARES program. - Problem Solving & Adaptability:
Identifies barriers to referrals or partnership engagement and responds with flexible, solution-oriented strategies in dynamic healthcare environments.
Frequent travel to provider offices and healthcare facilities in all regions CARES is located.
QUALIFICATIONS AND SKILLS- Bachelor’s degree in healthcare administration, public health, business, marketing, or related field (or equivalent experience).
- 3+ years of experience in healthcare sales, medical liaison work, or provider relations.
- Demonstrated success meeting performance targets or quotas.
- Strong presentation and communication skills.
- Experience working with healthcare providers and understanding clinical environments.
- Ability to work independently and manage a territory or outreach plan.
- Familiarity with EHR systems and referral workflows.
Must possess a valid CADL and applicable CA mandated auto insurance.
