JOB SUMMARY

The Intake Coordinator Mental Health Specialist/CM IV reports to the Supervising Mental Health Specialist and is responsible for providing coordination, development, and procurement of social service benefits for clients assigned to the program. This position will also include completing all new admissions in a timely manner with the goal of enrolling new clients for mental health services at the program. This position is expected to possess strong communication, rapport‑building, boundary‑setting, and problem‑solving skills.

This position will provide rehabilitation, case management, placement, group facilitation, and crisis intervention services. This position will focus on supporting clients with their individualized wellness goals by applying a collaborative person‑centered approach. This position provides support through rapport, compassionate care, education, role modeling, advocacy, skills building, and linkage to county benefits, vital documents, social, financial, medical, housing, and other services.

ESSENTIAL

JOB RESPONSIBILITIES
  • Under the supervision of the Supervising Mental Health Specialist, assist in the accomplishment of the case management plan with clients by screening, evaluating, identifying, and implementing specific case management activities as identified in the Individual Service and Support Plan (ISSP) to support clients in achieving their goals.
  • Collaborate with clients to develop an Individualized Service Plan (ISP). The plan will include written details of the supports, activities, and resources required for the individual to achieve their personal wellness goals. Special care will be taken to document the client’s personal choices/preferences, criminogenic, safety, financial, physical, mental, and behavioral health care needs.
  • Provide direct individualized services as outlined in the POC and ISP. May facilitate group services as needed under the direction of the Supervising MHS.
  • The Mental Health Specialist/Case Manager will be assigned a caseload of criminal justice involved adult clients living with Severe Mental Illness (SMI) and co‑occurring disorders.
  • Consistently meet program performance standards including a service impact of 50%, sign and submit documentation goals, and attainment of 40 hours of job‑related in‑service training per year.
  • Advocate and communicate closely with agencies, vendors, management, clients and significant others involved in the client care.
  • Attend daily team check‑out meetings, monthly one‑on‑one meetings with the Supervising MHS, and bi‑monthly team meetings, including staff meetings, probation case staffing, and high‑risk resource team (HRRT) meetings. Provide effective and thorough feedback regarding client engagement, progress, barriers, and needs.
  • Provide the Supervising MHS with timely reports and updates, including any key communications between community partners, vendors, clients, or unusual occurrences that might be relevant to program operation or the health and safety of clients or employees.
  • Exercise sound judgment when providing client services, prioritize safety, manage time efficiently to meet deadlines, and respond quickly/effectively in emergencies or pressured situations to support de‑escalation.
  • Interacts with program staff to schedule intake assessments.
  • Respond to referring parties upon receiving referral, meeting with clients, assessing desire for program, providing intake and coordinating/scheduling upon enrollment.
  • Meet directly with clients to complete initial admission documentation and enter new episodes in the EHR.
