Are you an experienced RN Case Manager in the Fresno, CA area looking for a new opportunity with a prestigious healthcare company? Do you want to advance your career by joining a rapidly growing company? If you answered “yes” to any of these questions, this is the position for you!

• Promoting a collaborative practice among the health care team by coordinating the integrated utilization management and discharge planning program.
• Monitoring, evaluating and implementing steps to improvement.

• Active CA RN license
• Previous Case Management experience is required
• Cerner experience is a plus
