Preventative Care Physician Assistant California's Valley

Seeking Physician Assistants and Nurse Practitioners who are passionate about community health and family medicine to perform Annual Health Assessments in Fresno and surrounding areas of California’s Central Valley.

• Full-time position
• Work with underserved and culturally diverse patient populations
• Modern facilities with state-of-the-art equipment
• Monday-Friday, 8 hour days

• Conduct Annual Health Assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
• Perform evidence-based practice screenings including point of care testing (as appropriate)
• Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
• Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of the assessment
• Communicate findings in the assessment that will be used to inform patients primary care provider of potential gaps in care
• Identify urgent and emergent situations and intervene appropriately
• Educate patients on topics such as disease process, medication, and compliance

• Salary starts at $137k/year plus bonus
