About the Position
Job Opportunity – Pediatric RN Case Manager – California
Start Date:

Compensation: Details provided to qualified applicants
Schedule:
40 hours per week | Monday through Friday, 8:00 AM to 4:30 PM | On-call rotation approximately 7 days per month
Highlights:
Provide specialized in-home hospice and concurrent care for pediatric patients from birth to 21 years
Manageable caseload with 1:15 ratio, averaging 4 to 5 home visits per day
4 to 6 week travel contract with consistent weekday schedule
Utilize Net Smart for documentation and care coordination
Focus on symptom management, skilled nursing assessments, and family education
Opportunity to make meaningful impact in pediatric hospice and concurrent care setting
Requirements:
Active California RN License
Minimum 2 years of pediatric experience in PICU, NICU, pediatric acute care, oncology, and/or hospice
Hospice or case management experience strongly preferred
Current BLS certification
Valid driver's license with reliable transportation
Current auto insurance with acceptable coverage and vehicle registration
