Hospice or case management experience preferred
Description:
Pediatric RN Case Manager providing in-home hospice and concurrent care services for patients from birth to 21 years.
Responsible for care coordination, skilled nursing assessments, symptom management, and family education.
Conduct 4 to 5 home visits per day.
On-call approximately 7 days per month.
Maintain a caseload ratio of 1:15.
Charting is done using Net Smart.
Reliable transportation is required.
A valid driver's license and current auto insurance with acceptable coverage are necessary.
Current vehicle registration is also required.
Candidates must confirm they are comfortable with home visits and an on-call rotation.
