The Clinical Manager is responsible for overseeing patient care coordination, clinical operations, and staff supervision in a home health setting. This role ensures high-quality, compliant care delivery, supports clinicians, and drives positive patient outcomes.
Key Responsibilities- Oversee patient care coordination and ensure care is delivered per physician orders and plan of care
- Develop, implement, and update individualized care plans in collaboration with physicians and interdisciplinary teams
- Review referrals, assess patient needs, and assign appropriate clinical staff
- Supervise and mentor clinicians to ensure quality patient care and regulatory compliance
- Conduct case conferences, chart reviews, and performance evaluations
- Guide clinicians on clinical decision‑making, goals, and treatment plans
- Ensure coverage and clinical support during all operating hours
Participate in quality improvement (QAPI) and performance programs - Assist with hiring, onboarding, and staff development/training
- Maintain compliance with agency policies, state and federal regulations, and professional standards
- Licensed Registered Nurse (RN) in California
- Home health experience required; leadership or supervisory experience preferred
- Strong knowledge of CMS regulations, care coordination, and clinical documentation
- Excellent communication, leadership, and organizational skills
