PositionRegistered Nurse (RN) – Hospice – Pay range: $50 - $65 per hour. Job type: per diem.
Job SummaryThe registered nurse plans, organizes, and directs home care services with an emphasis on community health education and support. The RN works collaboratively with patients, families, and interdisciplinary teams to develop individualized care plans and improve patients’ quality of life.
Duties & Responsibilities- Develops and initiates patient care plans, and coordinates interdisciplinary assessment and care in partnership with patients, representatives, and caregivers.
- Initiates preventive and rehabilitative nursing procedures and performs services ordered by the physician.
- Provides counseling to patients, caregivers, and families.
- Provides patient and caregiver education.
- Prepares clinical notes and documents.
- Communicates with physicians and other health care practitioners about the home health plan of care.
- Identifies discharge planning needs and implements plans prior to patient discharge.
- Graduated from an approved school of nursing and licensed in California.
- Maintains current CPR certification.
- Licensed driver with a reliable, insured automobile in good working condition.
- Self-directed with minimal supervision; excellent observational, problem‑solving, verbal, and written communication skills.
- Comprehensive nursing skill competency on the nursing competency checklist.
- Ability to plan, prioritize, and manage workload independently.
- Physical ability to walk, stand, lift, position, or transfer patients; lift supplies and equipment; reach, stoop, bend, kneel, or crouch; with adequate visual acuity and hearing.
