Registered Nurse (RN) – Home Health – Pay: $50.00 – $65.00 per hour – Job Type: Full Time
Job SummaryWe are seeking a compassionate and skilled Registered Nurse (RN) to join our team. This role involves planning, organizing, and coordinating home health services, with a strong emphasis on community health and patient education.
Key Responsibilities- Develop and initiate individualized care plans in collaboration with patients and caregivers
- Deliver physician-ordered nursing services and implement preventive and rehabilitative procedures
- Educate patients and caregivers to promote health and independence
- Identify and implement discharge planning needs
- Coordinate necessary equipment and services
- Supervise LVNs and HHAs
- Active California RN license
- Minimum 1 year of direct patient care experience (home health preferred)
- Current CPR certification
- Valid driver’s license and insured, reliable vehicle
