Primary Care Nurse Practitioner / Physician Assistant (Geriatrics) – Central California | Fresno Area Opportunity Highlights:
- Full-time Primary Care NP/PA position in an established PACE program
- Care for an older adult population using a collaborative interdisciplinary team model
- Comprehensive outpatient assessments and ongoing chronic disease management
- Coordinate care with physicians, specialists, therapists, and nursing teams
- Opportunity to build long-term relationships with a stable patient population
- Rotating evening and weekend call

- $150K–$190K salary plus annual bonus
