Primary Care Provider (Nurse Practitioner / Physician Assistant)
Salary: $150,000–$190,000 per year

Our client is seeking a compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join their PACE (Program of All-Inclusive Care for the Elderly) team in Fresno, California. PACE provides comprehensive medical care to seniors, helping them remain safely in their homes while receiving coordinated healthcare through an interdisciplinary team. As a Primary Care Provider, you will collaborate with physicians, nurses, therapists, social workers, dietitians, and other healthcare professionals to provide high-quality, patient-centered care.
Responsibilities- Perform comprehensive initial, annual, and follow-up assessments
- Diagnose and manage acute and chronic medical conditions
- Collaborate with physicians and interdisciplinary teams
- Coordinate care with hospitals, specialists, and nursing facilities
- Conduct nursing home visits as needed
- Participate in rotating evening/weekend call schedule
- Master's Degree from an accredited NP or PA program
- Active California NP License with Prescriptive Authority or California PA License
- Minimum 2 years of NP/PA experience
- Experience caring for geriatric or senior populations
- Excellent communication and teamwork skills
