Admission Registered Nurse; RN Per Diem - Hospice

The Registered Nurse (RN) plans, organizes, and directs/coordinates home care services with a focus on community health education. The RN builds on community resources to provide individualized care for patients and families within their homes and communities.

• Develop and initiate a plan of care and conduct ongoing interdisciplinary assessment.
• Provide preventive and rehabilitative nursing procedures as ordered by the physician.
• Give counseling to patients, caregivers, and families.
• Communicate with the physician responsible for the home‑health plan and with other health‑care practitioners.
• Identify discharge‑planning needs and implement plans prior to patient discharge.

• Graduate from an approved school of nursing and hold a California RN license.
• Maintain current CPR certification.
• Hold a valid driver’s license; possess an insured automobile in good working order.
• Be self‑directed, able to work with minimal supervision, and organize prioritization of workload independently.
• Exhibit excellent observation, problem‑solving, verbal and written communication skills, and nursing competency as per checklist.
• Be able to walk, stand, walk long distances, lift, position, or transfer patients and supplies.
• Perform physical tasks such as reaching, stooping, bending, kneeling, crouching, and lifting with visual and auditory acuity.

Pay Range: $55–$62 per hour.
