Delta-T Group Los Angeles, Inc. is seeking an RN Case Manager for per diem shifts in Fresno, CA. You will assess elderly clients, complete reassessment documentation, and conduct face‑to‑face assessments up to two hours each.
CA RN licensure is required; experience with older adult populations is preferred. Flexible Monday–Friday availability 6–16 hours per week, with part-time options. This is a temporary contract through Delta-T Group.
