Community Medical Centers is seeking a Registered Nurse (RN) Home Health Coordinator for Fresno, California.

- Responsible for the clinical review and assisting with the prioritization of visits for all patient referrals.
- Assists with the coordination of all aspects of patient care with home health clinicians, physicians, and any other patient care/service providers.
- Provides clinical support to the workflows in Community Home Care to ensure quality patient care.
- Graduation from an accredited School of Nursing or successful completion of sufficient units in Nursing to qualify for state licensure.
- One year of clinical experience as a Registered Nurse.
- Current California Registered Nurse license.
- Current BLS certification.
- Current California Driver License and driving record acceptable to Community Medical Centers.
