Oakwood Gardens Care Center is seeking a knowledgeable and dedicated Infection Prevention Nurse (LVN) to lead and coordinate our Infection Prevention and Control Program. This position plays a critical role in protecting the health and safety of our residents, staff, and visitors by ensuring compliance with infection control standards and regulatory requirements.

General Purpose

The Infection Prevention Nurse is responsible for planning, developing, implementing, monitoring, and evaluating the facility's infection prevention and control program. This position ensures compliance with CDC, OSHA, CMS, state, and local regulations while promoting best practices in infection prevention, employee health, and resident safety.

Essential Duties and Responsibilities Infection Prevention & Surveillance
  • Coordinate and oversee the facility's Infection Prevention and Control Program
  • Monitor infection control practices and ensure compliance with established policies and procedures
  • Conduct routine infection surveillance and track healthcare-associated infections
  • Investigate infection trends and implement corrective action plans as needed
  • Prepare monthly infection control reports and present findings to leadership and committees
  • Report communicable diseases to local and state public health agencies as required
  • Maintain infection surveillance logs and employee exposure records
Regulatory Compliance
  • Ensure compliance with CDC, OSHA, CMS, state, and local infection control regulations
  • Conduct rounds throughout the facility to monitor isolation precautions, sanitation practices, and infection control compliance
  • Participate in regulatory surveys and assist with survey preparation
  • Review laboratory reports and communicate findings to the interdisciplinary team
  • Develop and update infection control policies and procedures as needed
Education & Training
  • Provide infection prevention education and training to employees
  • Conduct orientation training related to infection control, hand hygiene, blood borne pathogens, and isolation precautions
  • Assist with annual OSHA, TB, and infection control in-service programs
  • Serve as a resource for staff regarding infection prevention practices
Employee Health
  • Assist with employee health programs including TB testing and exposure follow-up
  • Provide education regarding Hepatitis B vaccination programs and blood borne pathogen prevention
  • Maintain confidential employee health records and exposure documentation
Resident Care Support
  • Monitor residents on isolation precautions and ensure appropriate infection control measures are followed
  • Participate in resident assessments, care planning, and interdisciplinary meetings
  • Assist nursing staff with infection prevention-related clinical concerns
  • Obtain cultures and provide nursing support within scope of practice when necessary
Qualifications Education & Experience
  • Graduate of an accredited Vocational Nursing program
  • Minimum two (2) years of nursing experience preferred
  • Long-term care or skilled nursing experience preferred
  • Working knowledge of infection prevention, epidemiology, microbiology, and standard precautions
Licenses & Certifications
  • Current, active, and unrestricted California LVN license
  • Current CPR certification
  • APIC Certification preferred but not required
  • Must maintain all required continuing education and licensure requirements
Skills & Abilities
  • Strong organizational, communication, and leadership skills
  • Ability to interpret regulations, policies, and procedures
  • Experience with Point Click Care  (PCC) preferred
  • Ability to educate and train staff effectively
Physical Requirements
  • Ability to move throughout the facility throughout the workday
  • Ability to lift, push, pull, and carry up to 25 pounds
  • Ability to respond to emergencies and assist with resident evacuations if necessary
  • Ability to perform duties involving potential exposure to blood and body fluids
  • Ability to manage the physical and emotional demands of a healthcare environment
