Optum Home & Community Care is seeking a Nurse Practitioner to join our House Calls team in Tulare County. In this role, you’ll conduct in-home health assessments that help identify care needs, close gaps in care, and connect patients to appropriate resources, all while building meaningful one-on-one relationships.

- Conduct annual in-home health assessments
- Review medical history, medications, and health risks
- Perform physical assessments and preventive screenings

- Active and unrestricted Nurse Practitioner license, or the ability to obtain licensure in the state of assignment prior to start date.
- Current national certification through ANCC or AANP as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Adult-Gerontology Acute Care Nurse Practitioner (AGACNP), or Gerontology Nurse Practitioner (GNP), or the ability to obtain certification prior to start date.
- Ability to perform the physical requirements of the role, including lifting and carrying equipment up to 30 pounds, navigating stairs, and conducting assessments in a variety of home environments.
- Comfortable with prolonged periods of standing, sitting, bending, and kneeling while completing patient assessments.
- Reliable transportation and willingness to travel locally to conduct in-home patient visits.
- Strong communication skills and a commitment to delivering patient-centered care in a home-based setting.

Pay range and compensation package:The salary range for California residents is $109,500 to $164,000 per year. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, United Health Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
