Covenant House California (CHC) is a nonprofit organization that provides sanctuary and support for youth overcoming homelessness, ages 18-24. CHC believes that no young person deserves to be homeless; that every young person in California deserves housing, food, clothing, education, and most importantly, to be loved. As this position provides direct care to young people that are 18-24 years old, we require that applicants are at least 25 years old.

Position SummaryUnder the supervision of the Director of Community Housing and Alumni Support, the THP-Plus Case Manager is responsible for the evaluation, placement, and assessment of prospective and enrolled eligible young adults participating in the Transitional Housing Plus Program (THP-Plus). The target demographic includes young adults (18-24) who have been emancipated from foster care (including those supervised by the Juvenile Probation). The goal of the THP plus program is to offer housing and supportive services in a safe living environment, while helping participants develop the life skills needed to live independently.
Program participants are eligible to participate for 36 cumulative months or until the age of 25, whichever comes first.
THP‑Plus Case Manager will work with young people to ensure success in securing stable housing, employment, education, health and wellness, and circles of support or permanent connections. Other support services might include, but is not limited to: transportation assistance, development of daily living skills, counseling and mental health services, and obtaining vital documents.
The position is also responsible for providing these services within the framework of the Mission Statement, Five Principles, and the Policies and Procedures of Covenant House California.
Essential FunctionsProvide high quality support and develop positive relationships with residents and off‑site clients through effective utilization of the Covenant House California case management system; which includes:
Conducting Initial Intake
- Welcome youth to the program;
- Review initial intake documentation completed by receiving party
Completing a Full Intake Assessment
- Review and complete the Intake/Assessment within 3‑7 days and support the young person in identifying and accessing relevant resources.
Developing, Implementing, and Managing an Individual Service Plan (ISP)
- Success Plan (Housing Plan):
Developed within 30 days of intake and update every 30 days thereafter in both HMIS and ETO systems. - Wellness Survey:
Complete every 30 days. - Conduct regular home visits by driving to meet youth at their residences as required to provide case management support and ensure ongoing engagement - monthly for off‑site, and weekly for on‑site.
