POSITION SUMMARY:

The person in this position will provide case management services to HOPWA eligible individuals and their families to support their transition to self-sufficiency. Responsibilities include arranging, coordinating, monitoring, and delivering services that address clients’ housing needs and promote housing stability. Essential Job Functions include, but are not limited to, those listed below. Additional duties may be assigned as needed.

ESSENTIAL JOB FUNCTIONS:
  • Oversight of day-to-day operations of the program;
  • Build and maintain relationships with property owners/managers to secure units for leasing;
  • Meet with each client at least once weekly to provide support and assistance towards achieving each Individualized Plan;
  • Intake and assessment of incoming participants to determine eligibility and appropriate services;
  • Make referrals and appointments with appropriate agencies, services and/or case managers;
  • Maintain client files according to company policy and contract requirements;
  • Knowledge of the Clinical Data System and HMIS (Homeless Management Information System);
  • Provide daily, weekly, and monthly reports as requested;
  • Assist and monitor data collection and statistical reporting and complete Quality Improvement reports as requested;
  • Maintain relationships with referring and supporting agencies and represent the agency at various community meetings and outreach events as requested;
  • Coordination of services across multiple agencies;
  • Attend scheduled meetings and presentations with referring agencies;
  • Perform any other duties as assigned.

  • Excellent communication and public speaking skills.

  • Knowledge of substance abuse and treatment is desired.

  • Ability to exercise good judgment and discretion.

  • Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality.
  • Professional appearance and demeanor.
  • Ability to obtain and maintain clearance from LEIE, MSIL, and EPLS.
  • Valid driver’s license and ability to obtain and maintain West Care’s vehicle insurance is required.
  • Ability to obtain and maintain a successful background clearance.
  • During your tenure with West Care there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that West Care may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
  • CPR Certification, First Aid Certification, and an annual Tuberculosis test.
EDUCATION and/or

EXPERIENCE:
  • Demonstrate knowledge of housing services including one (1) year experience working in similar programs; working with the homeless population, substance use disorder and/or mental health care systems is required.
  • Demonstrate knowledge of target population including one (1) year experience serving people living with HIV is required.
  • A minimum of one (1) year experience implementing trauma-informed approaches is required.
  • A minimum of one (1) year experience with data collection, data analysis, and confidentiality is required.
  • Graduation from an accredited two-year college with a degree in Addiction Counseling, Counseling, Social Work, Psychology, or a related field is preferred.
  • High School Diploma or equivalent is required.

Not applicable for this position.

WORKING CONDITIONS:

Work is primarily performed in an office setting; and Some outdoor activities may be required.
