Position: FT Community Support Specialist

Calaim Community Support Specialist

Join our team at Resources for Independence Central Valley and play a vital role in empowering individuals with disabilities to transition successfully from institutional settings or homelessness to independent living within the community. The Community Support Specialist is under the direct supervision of the CalAIM IL Coordinator. The CS Specialist will work within the CalAIM Program to coordinate transition and diversion services for consumers referred to the program.

We are looking to hire for our Fresno Main office. As a CalAIM Community Support Specialist, you will:

  • Develop and cultivate strong partnerships with Landlords, Housing complexes, hospitals, rehabilitation centers, and other healthcare facilities to facilitate smooth transitions.
  • Collaborate with landlords and discharge planning staff to ensure individuals have the necessary services and supports in place prior to their transition.
  • Conduct comprehensive care plan assessments and eligibility screenings.
  • Provide direct support and advocacy to individuals throughout the transition process.
  • Coordinate and deliver essential goods and items to program participants.
  • Maintain accurate and up-to-date case files and documentation.
  • Enter data into the required Management Information Systems.
  • Participate in regular case staffing meetings.

Requirements:

  • Associate's degree in social work, human services, or a related field, or equivalent experience.
  • Minimum of three years of experience in case management, advocacy, or direct service within a social service setting.
  • Demonstrated ability to build rapport and establish trust with individuals with diverse disabilities.
  • Experience using electronic health record (EHR) systems, preferably MiCil.
  • Strong organizational and time-management skills, with the ability to manage a large caseload.
  • Ability to remain calm and effective in stressful situations.
  • Valid California driver's license and reliable transportation, with the ability to travel throughout the RICV service area.
  • Ability to lift, carry up to 20 lbs and to stand, walk for extended periods of time.
  • Deep understanding of and commitment to the principles of independent living and self-determination.
  • Excellent verbal and written communication skills, with the ability to present information effectively to diverse audiences.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
  • Strong problem solving and interpersonal skills.
  • Experience with receivable/payable and/or Medi-Cal billing is a plus.
